Submit attachments electronically

Submitting attachments for your claims and predetermination requests saves time, paper, and postage and is more secure than traditional mail. And it's easy! Simply go to https://emihealth.com and select Claim Attachments under the Providers tab. Complete the short form and attach your documents (not to exceed 500 MB) in pdf or jpg format. That's all there is to it! You may use any current browser, such as Microsoft Edge, Chrome, Firefox, Opera, or Safari. The form will not display in Internet Explorer 11.

We are offering this functionality through the Utah Health Information Network (UHIN), so if you happen to be logged into UHIN, you may also upload your attachments from there. Simply select EMI Health from the dropdown menu.
